Who We Are: USISA LLC (“USISA,” “we,” “us” or “our”) is headquartered in Omaha, NE.
Our Services: When we use the term “Services”, we are referring only to the Services that we offer on our own behalf, including the insurance technology products and services of Insurate Inc. and the websites that post or link to this Privacy Notice.
This Privacy Notice does not cover or address personal information and privacy practices relating to USISA job applicants, employees and other personnel.
1. What is Personal Information
When we use the term “personal information” in this Privacy Notice, we mean information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, to you. It does not include aggregated or deidentified information that is maintained in a form that is not reasonably capable of being associated with or linked to you.

From the first moment you interact with us, we are collecting personal information about you. Sometimes we collect personal information automatically when you interact with our Services and sometimes we collect personal information directly from you. At times, we may collect personal information about you from other sources and third parties, even before our first direct interaction.
Personal Information You Provide
We collect personal information you provide to us when you register for or otherwise use our Services on behalf of your business or employer, which may include:
Contact Information, including your name, email address, telephone number, and address when you register for our Services or engage in a transaction with us on behalf of your employer or business or otherwise communicate with us. We may also collect any communications between you and us, as well as any other information you provide to us.
Transaction Information, including payment information, credit information and order details when you purchase Services from us on behalf of your employer or business. Payments may be processed by third-party payment processors. Inquiry Information, including the content of emails or contact form submissions on our website, and, where applicable, voice recordings from calls with us when customers or prospective customers contact us.
Account Information, including your username and password.
Insurance Information, including health information and claim information submitted through applications and other forms about our customers’ injured workers and other employees, as well as information about other persons involved in or witnessing claims. For example, this may include type of injury, occupation, wages, employment history and social security number.
Professional/Employment Information, including your job title, department, employer or business.
Personal Information Automatically Collected
As is true of most digital platforms, we collect certain personal information automatically when you visit our online Services, including:
Log File Data, including your internet protocol (IP) address, operating system, browser type, browser id, date/time of visit, and pages visited.
Analytics Data, including the electronic path you take to our Services, through our Services and when exiting our Services, as well as your usage and activity on our Services, such as the links and object you view, click or otherwise interact with (also known as “Clickstream Data”).
Location Data, including your general geographic location based on your IP address or more precise location when accessing our online Services through a mobile device. For our marketing purposes. To help maintain the safety, security, and integrity of our Services, technology assets, and business.For our internal business purposes, such as data analysis, customer research, audits, fraud prevention, developing new products and/or features, enhancing the Services, improving our Services and identifying usage trends.To respond to law enforcement requests and as required by applicable law, court order, or governmental regulations and enforce our corporate reporting obligations and our Terms of Service, or to comply with applicable laws.For purposes disclosed at the time you provide your personal information or as otherwise set forth in this Privacy Notice.We may also use aggregate personal information in a manner such that the end-product does not uniquely identify you or any other user of the Services, such as to provide predictive modeling analytics. Within USISA: We may disclose personal information between and among USISA and its current and future parents, affiliates, subsidiaries, and other companies under common control or ownership. USISA entities disclose personal information to other USISA entities for purposes and uses that are consistent with this Privacy Notice.
Service Providers: We may disclose personal information with third-party vendors, consultants and other service providers who perform functions or services on our behalf. For example, we may share personal information with service providers for auditing, compliance, corporate governance functions, advertising and marketing, mailing services, and web hosting.
Business Partners: We may share personal information with third parties with whom we do business, including our channel partners who refer customers to us, in order to facilitate our Services, or for their own marketing purposes in compliance with applicable laws and subject to their separate privacy notices.
Insurers/Underwriters: We may share customer personal information with insurers/underwriters as necessary to provide you with our Services and pursuant to the terms of the relevant agreement(s) between us and our customers. What Information We Collect and How We Use That Information:
Please note that when you “opt-out” of receiving interest-based advertisements, this does not mean you will no longer see advertisements from us or on our Services. It means that the online ads that you do see from DAA program participants should not be based on your interests. We are not responsible for the effectiveness of, or compliance with, any third-parties’ opt-out options or programs or the accuracy of their statements regarding their programs. In addition, third parties may still use cookies to collect information about your use of our Services, including for analytics and fraud prevention as well as any other purpose permitted under the DAA’s Principles.
We participate in interest-based advertising and use third party advertising companies to serve you targeted advertisements based on your browsing history. We permit third party online advertising networks, social media companies and other third-party services, to collect information about your use of our online Services over time so that they may play or display ads on our Services, on other websites, or services you may use, and on other devices you may use. Typically, though not always, the information used for interest-based advertising is collected through data collection technologies, such as cookies, web beacons, embedded scripts, location-identifying technologies, and similar technology (collectively, “data collection technologies”), which recognize the device you are using and collect information, including click stream information, browser type, time and date you visited the site, AdID, precise geolocation and other information. 7. Children’s Personal Information
Our website and online Services are not directed to, and we do not intend to, or knowingly, collect or solicit personal information from children under the age of 13. If you are under the age of 13, please do not use our website or online Services or otherwise provide us with any personal information either directly or by other means. If a child under the age of 13 has provided personal information to us, we encourage the child’s parent or guardian to contact us to request that we remove the personal information from our systems. If we learn that any personal information we collect has been provided by a child under the age of 13, we will promptly delete that personal information.
